A Form 1099-G is a document sent by a federal, state, or local government agency to report certain income that may need to be reported on your return. Income listed on this form can include:
Unemployment compensation
State or local income tax refunds, credits, or offsets
Reemployment trade adjustment assistance (RTAA) payments
Taxable grants
Agricultural payments
Payments on a Commodity Credit Corporation (CCC) loan
If you received unemployment income, the amount of unemployment compensation should be reported in box 1 and the related federal taxes withheld in box 4. If state tax was withheld, that information would be reported in boxes 10a-11.
